Before your first appointment, you will be asked to complete a detailed intake form so we can learn more about your health history, lifestyle habits, and personal health goals. You will also be asked to complete a 3 day food journal to best illustrate your current eating habits. We ask that you send both of these to the dietitian at least 48 hours prior to your first visit.
On your first visit, we will review all of this information together and clarify any details as needed. By the end of your first visit, you will have a customized preliminary plan to get you started.
Translating nutritional science into practical solutions for improving your eating habits is only step one of the process. To ensure your success in maintaining lifestyle changes, we work collaboratively with you to create a plan and goals that best suit your individual circumstances.
Follow up appointments give you the opportunity to celebrate what is going well and to troubleshoot what is not working for you. In addition to building on your nutrition knowledge, we provide the guidance and encouragement that will keep you inspired and moving forward in your journey to better health.
